Reconstructive urology: rebuilding the urinary passage.

What is reconstructive urology?

Most urology removes something — a stone, an enlarged prostate. Reconstructive urology does the opposite: it repairs and rebuilds. It is called for when a passage is too narrow (a stricture), a junction is blocked (PUJ obstruction), or a bladder has become too small or high-pressure to store urine safely. These are planned, precise operations — often done with keyhole or robotic techniques.

Pyeloplasty — for PUJ obstruction.

PUJ (pelvi-ureteric junction) obstruction is a narrowing where the kidney meets the ureter. Urine drains poorly, the kidney stretches (hydronephrosis), and over time its function suffers. It is often found on an ultrasound — sometimes even before birth, sometimes after flank pain or infection in adulthood.

Pyeloplasty corrects it: the narrowed segment is removed and the junction rebuilt with a wide, smooth opening. It is usually performed by keyhole (laparoscopic) or robotic surgery — small cuts, short hospital stay, and quick recovery — and it protects the kidney from further damage.

Augmentation cystoplasty — enlarging a small, high-pressure bladder.

Some bladders — after long-standing obstruction, neurological disease or certain infections — become small, stiff and high-pressure. Left alone, the pressure travels up and damages the kidneys. When medicines are no longer enough, augmentation cystoplasty enlarges the bladder using a carefully prepared segment of intestine, lowering the pressure and restoring safe storage capacity.

It is a considered, planned operation offered after thorough evaluation (including bladder pressure studies), and patients are guided closely before and after — including learning clean intermittent catheterisation where needed.
